COVID-19 Reporting

COVID-19 SELF REPORTING FORM FOR STUDENTS

Check your temperature and COVID-19 related symptoms before returning to campus and before coming to campus each day.

If you have any of these symptoms, DO NOT come to campus.

  •  Employees or students who have tested positive or who have symptoms of COVID-19 should seek medical care, notify their supervisor (in the case of employees), stay home, and complete the COVID-19 Self-reporting Form (students).
  • People with COVID-19 have had a wide range of symptoms reported – ranging from mild symptoms to severe illness. These symptoms may appear 2-14 days after exposure to the virus:
    • Fever over 100.4°F or 38°C (without having taken any fever-reducing medications)
    • Loss of smell or taste
    • Cough
    • Muscle aches
    • Sore throat
    • Shortness of breath
    • Chills
    • New or unusual headache
    • Nausea, vomiting, diarrhea, or loss of appetite
  • Review latest symptoms from CDC

If the answer is “yes” to one or more of the following questions, you   should stay home.

  • Are you experiencing any of the COVID-19 related symptoms noted above?
  • Are you living with or caring for an individual who is suspected or confirmed case of COVID-19?
  • Have you been in direct or close contact with anyone known or suspected to have COVID-19 in the last 14 days?
  • Have you tested positive for COVID-19 since your last screening?
  • Have you been asked to self-isolate or quarantine by a medical professional or a local public health official

If you are experiencing symptoms associated with COVID-19, you should not come to campus.

SGSC students should fill out the self reporting form for students found on this page. Employees should report COVID-19 exposures, symptoms, or confirmed cases to SGSC Human Resources.

If you believe that you are experiencing symptoms of COVID-19 or have been exposed to it, please contact your healthcare provider. If you are having chest pain, serious difficulty breathing, or another type of medical emergency, please call 911. The State of Georgia has a COVID-19 hotline: (844) 442-2681. You may call this number for additional screening and testing information.

Affected employees and students may return to campus when they have met all three of the following criteria:

  • The individual has no fever for at least 72 hours without the use of fever-reducing medication, AND
  •  Other symptoms have improved, AND
  • At least 10 days have passed since symptoms first appeared.

South Georgia State College’s goal is to promote a healthy and safe environment; comply with Executive Orders and directives from the Governor’s office; and reflect guidance from the Georgia Department of Public Health (GDPH) and the Centers for Disease Control and Prevention (CDC) recommendations. We also recognize the COVID-19 situation is fluid and guidance will likely continue to change.

It is to be noted that everyone on campus has an individual responsibility to contribute to proper behavior and to adhere to GDPH guidance in order to protect themselves and others as well as to help contain the spread of the virus. Institutional controls are only as effective as the willingness of individuals to carry them out. It is essential that everyone on campus take responsibility for their actions and adhere to the guidelines.
